My question is he finally said he wants help but it was as I was leaving him. I want to help him so we can get our family back together but I don't know what to do or how to do it.

For alcoholism and something highly addictive like crack cocaine you should encourage him to go to a drug and alcohol rehab.

Use the Substance Abuse Treatment Facility Locator by the US Department of Health and Services to help him find a proper addiction treatment program. It covers programs across a wide variety of spectrums and also allows you to find programs that are partially or totally subsidized if cost is an issue.

There is loads of professional help available out there to help him get started on a new life so with a bit of research I'm sure you'll find a good program for him.
